911

911 is an emergency service routed to your 911 service address only and available only by using approved stationary equipment connected to approved conductivity at that address. 911 service provided with Qwest^® Broadband Phone Service is not traditional 911 service and there may be situations where 911 is NOT AVAILABLE. Carefully review the "911 Emergency Service" section of your Terms and Conditions for a more thorough explanation of the limitations to 911 service.

Circumstances in which 911 services are not available at your Service Address include, but are not limited to:

• Power failure at your location
• Degraded or down Internet or Broadband connectivity
• Incorrect installation of your Broadband phone service adapter
• Broadband Phone Service not working properly
• Cable/DSL modem, Broadband Phone service Adapter or other related equipment malfunctions
• Misusing the Broadband Phone Service at a location other than your current Service Address (an unapproved location) may cause your 911 calls to be routed to the incorrect Public Safety Access Point (PSAP) and will provide incorrect address information to the 911 operator

If you desire to move your service and use it at a location DIFFERENT from your Service Address or desire to permanently change your Service Address, you must submit your new Service Address at www.qwest.com/mybroadbandphone or contact Qwest at 1 866-283-0043 (or 1 877-878-7543 after business hours) and obtain Qwest's approval. You will receive a message indicating whether Qwest can accept or reject your proposed address. Requests to modify your Service Address may take approximately 15 minutes or up to 72 hours from when the request was submitted, depending on the type of 911 service available at the proposed Service Address. Any 911 calls made during that period will route emergency services to your current address, not your proposed address. Qwest will e-mail you at your e-mail address of record when 911 service is available at the new, approved address.

If your request involves transferring your number to the new location, in certain circumstances, establishing 911 service may take more than 30 days. Contact Qwest for details.

NOT ALL ADDRESS CHANGES CAN BE APPROVED. Qwest will only approve addresses where 911 services can be provided. Failure to obtain approval is prohibited by the Terms and Conditions of Broadband Phone Service and constitutes a misuse of the service. Such misuse will route your 911 calls to the incorrect 911 operator and provide incorrect information to the 911 operator. If you request a permanent change for your Service Address to a location where 911 services are not available, Qwest will disconnect your Broadband Phone Service.

Caller ID

Don't miss an important call or take another unwanted one. Caller ID allows you to identify the caller before you pick up the phone (requires separate caller ID display unit or caller ID capable phone). You can block the appearance of your number to Caller ID subscribers whenever you do not want it displayed.

• Caller ID Number - Displays incoming number (listed telephone number of the line from which the call is being placed).
• Stores number, date and time of most incoming local and long distance calls whether you take the call or not

Note: Names and numbers from some calls may not display.

Call Logs

View incoming, outgoing and missed calls from the last 30 days. Call logs display the phone number, date and time of the call and the caller's name (if available).

Call Waiting

Don't miss important calls while on the phone. A special tone alerts you to an incoming call. Press the switchhook (or flash) to answer the call while your other caller stays connected.

Your Call Waiting ID display Unit will show you the second incoming caller and number (if available) so even if you choose not to answer, the call information is stored on your Call Waiting ID display Unit, with compatible customer equipment.

If you don't want to be interrupted during a call, temporarily turn off call waiting by dialing *70 before you make the call or turn the feature off in the MyBroadbandPhone account management portal.

Click-to-Call

Make a call directly from your call log. Simply click on a phone number to dial.

Custom Ringing

Custom Ringing allows you to program incoming phone numbers so they ring distinctively when they are calling from the identified numbers. You can add or delete numbers at any time from your MyBroadbandPhone portal.

Do Not Disturb

Sometimes you want to be available, other times you'd rather not be disturbed. Program your line to block all incoming calls without affecting outgoing calls.

FlexMail™

FlexMail™ is similar to traditional voice mail, but has many more capabilities. Like voice mail, your unanswered calls can be forwarded to FlexMail where your callers can leave you a message. Unlike voice mail, you can be notified of and access messages in several ways.

You can be notified of messages by:

• A stutter dial done on your phone
• A message waiting indicator light on your phone, if applicable
• E-mail
• MyBroadbandPhone online portal

You can also access your messages in many ways:

• Dial your access number and enter your password.
• Listen to them online from the MyBroadbandPhone portal.
• Listen to them in an e-mail message, which can be forwarded to any e-mail account, as well as multiple email accounts.

You choose your notification preferences from MyBroadbandPhone portal.

3-Way Calling

Like a conference call, you can talk to two different people in separate locations at the same time. Simply place one caller on hold while you place a second call by pressing the FLASH key or switch hook button. Press the FLASH key or switch hook button again to connect all three parties.

Outgoing and Incoming Call Block

Identify and block certain numbers from calling you, including private or anonymous numbers and block outgoing calls to directory assistance and international numbers.

Speed Dial

Calling your most important phone numbers can be as simple as dialing two digits. Speed Dial lets you program numbers for fast and easy access. Within the MyBroadbandPhone portal, simply make a list of up to 20 numbers you want to program and assign each one a two-digit code. Then just dial the two digit code followed by the # sign and your call will be placed.

Unlimited Local and Domestic Long Distance

Make unlimited local and domestic long-distance calls.

Reach Me

Reach Me forwards your calls to any Domestic phone numbers, so callers can reach you anytime, anywhere. Or choose only those callers that you want to be forwarded. You can send calls to a variety of numbers in an order of priority you choose, up to 5 numbers. Program forwarded calls so they ring to you sequentially or simultaneously.

Selective Reach Me

Selective Reach Me forwards your calls to any Domestic phone numbers, so callers can reach you anytime, anywhere. Or choose only those callers that you want to be forwarded. You can send calls to a variety of numbers in an order of priority you choose, up to 5 numbers. Program forwarded calls so they ring to you sequentially or simultaneously.

MyBroadbandPhone

All Qwest Broadband Phone Service accounts come with MyBroadbandPhone, a convenient way to manage your account and features online—see your phone calls, listen to voice mail, view call logs, manage your features and more.

Virtual Number

For a nominal fee, choose up to five additional phone numbers (virtual numbers) that will ring to your phone. Friends and family who would normally pay long-distance charges when calling you can call your virtual number instead (located in their local calling area) for the price of a local call.

International Long Distance

Directory Assistance

Directory Assistance allows you to search for numbers and addresses in nationwide and local directories. You don't need to know the area code to do your search. Directory Assistance will find it for you.

All Qwest OneFlex accounts come with MyOneFlex, a convenient account management feature accessible anywhere, anytime, from any Internet connection. With MyOneFlex, you can access your OneFlex^® account, see your phone calls, manage your features and more.

Domestic Long-Distance

Just 5 cents a minute for all your direct-dialed domestic long-distance calls with a $2 monthly recurring charge per line and $0.99 Interstate service fee per account. Pay no more than $19.99 a month in long-distance charges per phone line. All monthly fees apply toward the $19.99 cap.
